Duties/Responsibilities:
· Utilizes knowledge of clinical diagnostic methods and principles to assess patient status and acute needs.
· Upon service initiation, ensure that each client has an initial service plan which addresses the client’s immediate health, safety, and preliminary service needs.
· Perform data entry/updates for assigned clients into Minnesota Department of Human Services on DAANES.
· Complete a comprehensive assessment and an assessment summary of the client's chemical use problems within three calendar days after service initiation. The assessment summary must be formatted utilizing the ASAM criteria.
· Provide an Individual Treatment Plan after completion of the comprehensive assessment which is subject to amendment until services to the client are terminated. The individual treatment plan must include treatment goals, specific methods to be used to address identified problems, resources to which the client is being referred, and client goals.
· Enter progress notes in each client’s file weekly or after each treatment service. The progress notes must reference the individual treatment plan.
· Complete a discharge summary within five days of the client’s termination of services.
· Ensure the content and format of client records be uniform and entries in each case are signed and dated.
· Provide individual and group counseling, client education strategies to avoid inappropriate chemical use and health problems related to chemical use, transition services, and services to address issues related to co-occurring mental illness.
· Provide culturally sensitive services to diverse clients and their families.
· Participate in the weekly treatment team meetings to review and update client progress notes.
· Initiate and maintain ongoing communication with client referral, probation, social service agencies, etc.
· Provides crisis intervention, when needed, based on proven and researched intervention approaches.
· Provides treatment in a manner that reflects knowledge of a range of treatment interventions and methods and demonstrates that knowledge through documentation of those interventions and methods in the record of the client.
· Complete all intakes and admissions
· Obtain insurance authorizations as needed.
· Ensure compliance with DHS regulations.
· Complete documentation in Procentive electronic health record.
· Perform other duties as assigned.
Education/Experience/Qualifications:
· Meet the qualification requirements for an alcohol and drug counselor (LADC or ADC-T)
· ADC-T may be considered depending on experience. Co-occurring license Preferred.
· 3+ years of experience providing individual and group counseling to individuals with substance use disorder
· Must be free of problematic substance use for at least the two years immediately preceding employment
· Know and understand the implications of chapters 245G.01-245G.22 chapter 245A.65, and sections 626.556, 626.557, and 626.5572.
· Pass a background check through the Minnesota Department of Human Services.
· Must have First Aid/CPR certification or be willing to obtain within 30 days of hire.
· Experience and competency in working with individuals and families from diverse backgrounds.
Compensation: $25.00 - $30.00 per hour
